Call For Applications

C4 aims to release our 10th grant cycle of the Community Grants, if funding allows. We are launching our C4 Screen Your Gut – Save Your Butt 5k Challenge in San Diego on March 9, 2024, which we hope will establish additional funding for the C4 Community Grants program. If you would like to help us raise funds, we are offering both in-person and virtual participation by individuals or teams. If you are part of a Federally Qualified Health Center or Community Health Center, you might want to start a team representing your FQHC.

The 2024 Community Grants application cycle is now open and can be accessed here. You can directly download the RFA or access from within the application portal.

Applications are due by January 5, 2024 at 11:59pm (midnight) (PT) and funding decisions will be distributed the last week of February.

All projects will start on March 1, 2024 following the completion of an acceptable MOU. The budget per project is limited to $10,000 per award, assuming adequate justification of expenses. The RFA includes information on the applicant training session, which requires pre-registration and will be held on November 29, 2023 at noon (PT).
