2023 COMMUNITY GRANTS
C4 aims to release our 9th grant cycle of the Community Grants, if funding allows. We are launching our C4 Screen Your Gut – Save Your Butt 5k Challenge in San Diego on March 4, 2023, which we hope will establish additional funding for the C4 Community Grants program. For now, the 2022 grants are currently ongoing and a release date for new funding for 2023 has not yet been established. If you would like to help us raise funds, we are offering both in-person and virtual participation by individuals or teams. If you are part of a Federally Qualified Health Center or Community Health Center, you might want to start a team representing your FQHC.
New! Breast and Colorectal Cancer Screening Implementation Project (BCCSIP)
C4 has established a new project in partnership with the Department of Health Care Services (DHCS)/Every Woman Counts (EWC) program and the CA Department of Public Health/Comprehensive Cancer Control. The focus of the project is on adding evidence-based interventions to existing breast and colorectal cancer screening initiatives within Federally Qualified Health Centers (FQHC) with the aim of increasing screening rates for both cancers. The funding amounts will be for up to $15,000, depending on budget justifications and are restricted to FQHC that are also Every Woman Counts providers.
Applications open on February 17, 2023 and are due no later than 5 PM (PT) on March 29. To access the application portal, click here.
