2013 Community Grants

In 2013, C4 awarded $22,000 for the following grants:

  • L.A. Care Health Plan – $5,000. This partnership is a community-based managed care system for low-income and vulnerable populations in Los Angeles County.
  • San Diego Neighborhood Healthcare – $5,000. This partnership piloted a colorectal cancer program to increase the dissemination and return of FIT tests in San Diego County.
  • University of California, Davis – $7,500. The funding went towards expansion of the UC Davis WeCare! Peer Navigation program to include training and use of peer navigators for patients newly diagnosed with colorectal cancer.
  • University of California, San Francisco – $4,500. The project involved translating patient education videos for FIT for use with patients in primary care clinical settings.

The community grants are funded in part by a generous donation from the Colon Cancer Alliance through the Undy Walk/Run programs (Sacramento and San Diego) in California.
